Formula

1
HEART = History + ECG + Age + Risk factors + Troponin
2
Each component: 0, 1, or 2 points.

How it works: The HEART score stratifies short-term major cardiac event risk among patients with chest pain. Low scores support outpatient pathways in appropriate low-risk cohorts; higher scores warrant observation and cardiology input.

Worked Example

Highly suspicious history (2), nonspecific ECG (1), age 50 (1), two risk factors (1), normal troponin (0): total HEART = 5.

HEART score and ACS

The HEART score helps emergency clinicians align chest pain evaluation with objective risk. It does not replace electrocardiographic STEMI recognition or institutional chest pain protocols.

  • Low scores (often 0 to 3) may be compatible with early discharge in selected patients with negative workup
  • Scores 4 to 6 suggest observation and repeat biomarkers
  • Scores 7 to 10 indicate high short-term event risk and usually invasive evaluation
  • Always apply your hospital exclusion criteria before using any score for discharge

Serial high-sensitivity troponin protocols may update risk after the first HEART score. Document shared decision-making.

You can also calculate changes using our ASCVD Risk Calculator, CHA2DS2-VASc Score or HAS-BLED Score.

Frequently Asked Questions

What does heart score calculator ACS mean?

ACS means acute coronary syndrome. The HEART score was developed for patients with possible ACS symptoms such as chest pain or equivalent anginal symptoms.

Can I discharge at HEART 3?

Some studies support outpatient management for low HEART scores with negative serial troponins and normal vitals. Your hospital policy and cardiology backup define eligibility.

Does a normal first ECG rule out MI?

No. Acute coronary syndromes including NSTEMI may present with a non-diagnostic ECG. Troponin trajectory matters.

How do I score troponin if only one level exists?

Use the category that matches your lab relation to the upper reference limit. If indeterminate, repeat testing per protocol.

Is HEART validated everywhere?

External validation varies by country and population. Use it as one input among history, exam, ECG, and imaging.

What about cocaine or PE as cause of chest pain?

Alternative diagnoses can mimic ACS. HEART does not replace consideration of pulmonary embolism, aortic dissection, or toxicologic causes.
