Formula
How it works: Combines the insulin needed to bring down high blood sugar and the insulin required to cover a meal.
Worked Example
Insulin Dose Calculator - Bolus from Carbs and BG
A mealtime insulin bolus combines two components: a correction dose to bring high blood sugar back to target, and carb coverage to handle the carbohydrates in your meal. This calculator adds both components for a total dose estimate.
- Total Dose = Correction Dose + Carb Coverage
- Correction Dose = (Current Blood Sugar - Target) / Correction Factor (ISF)
- Carb Coverage = Grams of Carbohydrates / Insulin-to-Carb Ratio (ICR)
- Typical starting total daily dose (TDD): 0.5 to 1.0 units per kg of body weight
- Rapid-acting insulin onset: 15 minutes. Peak: 1 to 2 hours. Duration: 4 to 5 hours
This calculator provides estimates only. All insulin doses should be confirmed with your prescribing physician or diabetes care team.

Frequently Asked Questions
What is the Correction Factor?
The Correction Factor (also called Insulin Sensitivity Factor or ISF) is how many mg/dL one unit of rapid-acting insulin will lower your blood sugar. It is estimated by dividing 1800 by your Total Daily Dose (TDD). For example, TDD of 36 units gives a correction factor of 50.
What is the Carb Ratio?
The Carb Ratio (ICR) is the number of grams of carbohydrates covered by one unit of insulin. It is estimated by dividing 500 by your TDD. A TDD of 50 gives a ratio of 1:10, meaning 1 unit covers 10 grams of carbs.
Is this calculator for all insulin types?
This logic applies to rapid-acting insulin analogs (lispro, aspart, glulisine) and regular insulin used for mealtime boluses. It does not apply to long-acting basal insulin like glargine or detemir.
What is a typical Total Daily Dose?
Most adults with Type 1 diabetes use 0.5 to 1.0 units per kilogram of body weight per day. For a 70 kg person, that is 35 to 70 units total, split between basal and bolus doses.
How do I avoid insulin stacking?
Wait at least 3 to 4 hours between correction doses. Rapid-acting insulin remains active for 4 to 5 hours, and giving additional doses too soon can cause dangerous hypoglycemia (blood sugar below 70 mg/dL).
Should I skip the correction if my blood sugar is at target?
Yes. If your current blood sugar equals your target, the correction component is zero. You would only dose for carb coverage in that case.
Can I use this Insulin Dose Calculator on my own web page?
