Calcium in the blood is about 40% bound to proteins, primarily albumin. When albumin levels are low (hypoalbuminemia), the total measured calcium may appear low even if the active (ionized) calcium is normal.
- Formula: Corrected Ca (mg/dL) = Measured Ca + 0.8 x (4.0 - Albumin in g/dL)
- Normal total calcium range: 8.5 to 10.5 mg/dL
- Use when serum albumin is less than 4.0 g/dL to avoid false hypocalcemia diagnosis
- For every 1 g/dL albumin decrease, add 0.8 mg/dL to measured calcium
- Ionized calcium (1.12 to 1.32 mmol/L) is the gold standard for direct measurement
